Change log

The change log records all requests for change and their progress through the change control procedure.

Typical headings in the log include:

  • Identifier
  • Each change request should have a unique identifier. This is primarily used for cross-referencing in reports and supporting documentation.

  • Date
  • The date the request was first received by the management team.

  • Requested by
  • The name and details of the stakeholder making the change request.

  • Description
  • A description of the change being requested. This may be the request in full or may reference more detailed specifications of proposed changes to a product or products.

  • Status
  • The current status of the request within the change control procedure and predictions of when the procedure will be completed.

  • Decision
  • A note of whether the request was accepted or rejected and a summary of the reasons why.

  • Date closed
  • The date when the decision was reached.

  • Communication
  • Confirmation that the decision has been communicated to all relevant stakeholders with feedback where appropriate.

  • Cross references
  • Cross references could identify detailed information about the nature of the request or about the assessment process. It is useful for assurance purposes to indicate which delivery plans that have been updated to reflect any accepted change requests.
